Every bleed? Part 2: beyond the trauma bay.

Six more TXA calls: GI bleeding, nosebleeds, hemoptysis, heavy periods, dental work on anticoagulants and inherited bleeding disorders — plus what it can cost.

Every bleed?

Corrected October 2026 — the slides call TXA in variceal bleeding an open question. Baveno VIII (2026) recommends against it, and this page now says so.

Part 2 · beyond the trauma bay. Make your call on each — give or don't — before reading on. The answers are on card 8.

Your call

  • Upper GI bleed, BP 85/50
  • Nosebleed still going after pressure
  • Hemoptysis, a few spoonfuls a day
  • Molar extraction on apixaban
  • Heavy periods, trying to conceive
  • Mild type 1 VWD, VWF 0.45, tooth out

From part 1

TXA protects clot that has already formed. It works where fibrinolysis drives the bleeding.

TXA II · 2 of 9

GI bleeding: no survival benefit, more clots.

Small older trials suggested fewer deaths. The largest trial found none, and venous thrombosis doubled.

HALT-IT 2020 · 12,009 upper and lower GI bleeds — patients, %

TXAPlacebo
Death from bleeding — 5 days · RR 0.993.73.8
Venous clots — RR 1.85 · seizures 0.6% vs 0.4%0.80.4

Who was enrolled

Dose1 g over 10 minutes, then 3 g over 24 hours. Twice the total trauma dose.
Liver diseaseAlmost half had suspected variceal bleeding, and they accounted for nearly 3 in 4 deaths.
TimingOver 80% were treated more than 3 hours after bleeding began.

What the guidelines say

ESGE 2021Recommends against TXA in nonvariceal upper GI bleeding.
ACG 2023Strongly recommends against antifibrinolytics in lower GI bleeding.
Baveno VIII 2026Recommends against TXA in acute variceal bleeding (statement 5.25, strong).
Varices: guidance now says no — Baveno VIII (2026) recommends against TXA in acute variceal bleeding (statement 5.25, strong). HALT-IT: no benefit, and the venous clot excess was concentrated in liver disease (14 vs 2 events, exploratory). Kumar, 2024: 600 with Child B or C cirrhosis; failure to control bleeding by day 5 fell, 6.3% vs 13.3%, mostly from band ligation sites. Mortality unchanged.

Heavy periods: a nonhormonal option.

Fibrinolysis is increased in the endometrium of women with heavy periods. Oral TXA, taken only on bleeding days, cuts blood loss by about 40%.

Lukes 2010 · menstrual blood loss, change from baseline (target −50 mL)

TXA — 1.3 g 3 times daily · 115−69.6 mL (−40.4%)
Placebo — 72 women−12.6 mL (−8.2%)

How it is used

Dose1,300 mg orally 3 times a day for up to 5 days, during menstruation only.
RankingSecond to the hormonal IUD in a 2023 network meta-analysis; ahead of NSAIDs and oral progestogens.
US labelContraindicated with active or past thrombosis or an intrinsic clotting risk.
VWD 2021Hormonal therapy or TXA over desmopressin; TXA if trying to conceive.
Where it fits — it does not treat the cause: fibroids, polyps and endometrial pathology still need workup. It has no hormonal effect, so it suits patients trying to conceive. Combined hormonal contraception adds clot risk.

Nose and lungs: small trials.

Topical and inhaled TXA reach the bleeding surface with little systemic exposure. The one large trial, in nosebleeds, was negative. The lung trials are positive but small.

Key outcome, TXA against placebo — patients, %

TXAPlacebo
NoPAC 2021 — nose · needed packing43.741.3
Wand 2018 — lung · resolved, day 59650
TXA-NEB 2025 — lung · resolved63.024.1
NosebleedsCompared with standard packing, a TXA-soaked pledget stopped bleeding within 10 minutes in 71% vs 31% (Zahed 2013; 73% vs 29% on antiplatelets, 2018). These were small, open-label trials. Unlike NoPAC, they did not compare TXA with placebo.
HemoptysisNebulized 500 mg 3 times daily. In Wand, invasive procedures 0% vs 18%. Massive hemoptysis and unstable patients were excluded.
Why the question stays open — pooled data disagree with NoPAC: a 2022 meta-analysis found early hemostasis 3.5 times as likely with topical TXA. The lung trials are single-center: a 2025 retrospective study found no clear benefit and possible harm.

Tooth extraction on anticoagulants.

The usual plan is to continue the anticoagulant, time the procedure around it, and control bleeding locally. TXA mouthwash is an add-on.

EXTRACT-NOAC 2021 · the mouthwash schedule

One rinse before (day 0), then 3 rinses a day through day 3; bleeding checked at day 7. Morning DOAC dose skipped, so extraction falls at trough.

Patients who bled · primary26.4% vs 28.6%. 218 patients on DOACs. RR 0.92, not significant.
Total bleeds46 vs 85. Rate ratio 0.57, not significant. Fewer unplanned visits: rate ratio 0.39.

What the guidance says

SDCEP, UKDoes not advise primary care dentists to prescribe TXA: off-label, and may not beat other local measures.
WarfarinA 2-day course of 4.8% mouthwash worked as well as 5 days in 85 patients.
Where it may help most — several teeth at once: bleeding after multiple extractions appeared lower with TXA. Late oozing: delayed bleeding also appeared lower. Both were secondary outcomes.

Inherited bleeding disorders.

Mucosal bleeding from the mouth, nose and uterus is where TXA helps most in von Willebrand disease and hemophilia, usually alongside factor or desmopressin.

Von Willebrand disease · ASH/ISTH/NHF/WFH 2021

Minor mucosal procedureType 1, VWF above 0.30 IU/mL, mild bleeding history? Yes — TXA alone. No — VWF to 0.50+ plus TXA.
Heavy menstrual bleedingTrying to conceive? Yes — TXA over desmopressin. No — hormonal therapy or TXA.

Hemophilia and practical points

US IV labelHemophilia, short-term use for 2 to 8 days around tooth extraction.
RoleAn adjunct to factor replacement or desmopressin, not a substitute in severe disease.
AvoidBleeding from the upper urinary tract, where clot can obstruct the ureter.
How strong is the evidence? — weak on paper: all four VWD recommendations above are conditional, on very low-certainty evidence. Under review: a 2025 monitoring report reassessed the guideline to decide whether it needs revision.

What it can cost.

At trauma and obstetric doses, harms are rare. They rise with dose, with late use, and with kidney failure.

SeizuresBlocks inhibitory GABA-A and glycine receptors. Above 2 g a day, risk roughly tripled in a meta-analysis. Most often after cardiac surgery.
Kidney failureCleared by the kidney. Give the first emergency dose without waiting for creatinine; reduce later doses.
Wrong routeIntrathecal injection, mistaken for a spinal anesthetic, causes seizures, arrhythmia and death. Vials look alike.

Dose and the clot signal — total IV dose

WOMAN (postpartum)1 g · no clot signal
CRASH-2 (trauma)2 g · no clot signal
POISE-3 (noncardiac surgery)2 g · heart events unclear
HALT-IT (GI bleeding, given late)4 g · VTE 0.8% vs 0.4%

What the labeling says

US labelContraindicated for neuraxial use, in subarachnoid hemorrhage and in active intravascular clotting. IV use only.
FDA 2025Boxed warning on wrong-route errors. Store TXA away from spinal and epidural drugs.
Hematuria from the upper tract — TXA can cause clot retention and ureteric obstruction.

Deciding, and the answers.

Start with what is bleeding. For trauma and birth, ask how long ago it started. Surgery is in part 1.

Decision path — what is bleeding?

Trauma or birthUnder 3 hours? Yes — IV TXA now. No — don't start.
Mucosa: mouth, uterus, airwayTopical, oral or inhaled TXA.
GI tractDon't give routinely.

Card 1 answers

Don'tUpper GI bleed: HALT-IT, no benefit and more venous clots.
UnprovenNosebleed after pressure: NoPAC negative, older trials positive.
ConsiderNonmassive hemoptysis: nebulized TXA, small positive trials.
ConsiderExtraction on apixaban: mouthwash as an add-on to local measures.
GiveHeavy periods, trying to conceive: oral TXA, no hormonal effect.
GiveType 1 VWD, VWF above 0.30, minor mucosal procedure: TXA alone.

Oral, topical and inhaled doses.

Adult doses from the trials and labels in this post. Topical and inhaled use is off-label.

Oral

Heavy periods1,300 mg 3 times a day, up to 5 days per cycle.

Topical and inhaled

Dental, DOAC10% mouthwash once before, then 3 times a day for 3 days.
Dental, warfarin4.8% mouthwash; 2 days worked as well as 5.
HemoptysisNebulized 500 mg 3 times a day in trials.
Nosebleed500 mg on a cotton pledget in Zahed; 200 mg, repeated once, in NoPAC, which was negative.
Before prescribing — off-label routes: mouthwash and nebulized doses come from trials, not product labels. Check the kidneys: oral doses also need reducing as clearance falls.
