How IM fellowships match

Internal medicine subspecialties apply through ERAS in the July cycle and match through the NRMP's Medicine and Pediatric Specialties Match. That match replaced the Medical Specialties Matching Program (MSMP) starting with the 2022 Match. Its IM fellowships include cardiology and its subspecialties, critical care, endocrinology, GI, geriatrics, hematology and oncology, ID, interventional pulmonology, nephrology, pulmonary and PCCM, and rheumatology. Most residents apply in July at the start of PGY-3.

Letters that carry weight.

ERAS rules

  • You can assign up to four letters to each program.
  • Letter writers upload through the ERAS Letter of Recommendation Portal. You cannot upload them yourself.
  • Following up with writers is your job.

Who to ask

Program directorAAIM publishes guidelines for PD letters for subspecialty applicants and encourages programs to use them. Plan on one of your four.
Faculty in the fieldOne or two who have supervised you clinically and can describe specific patients and decisions.
Research mentorWorth including if your work is in the field or shows you finish what you start.

Make it easy to write a strong letter

  • Ask early: about two months before July 1.
  • Ask directly: “Do you feel you can write me a strong letter?”
  • Send your CV, a draft statement, your career goal, and two or three cases you worked on together.
  • Give a soft deadline two weeks before you plan to submit. Send one polite reminder.

A statement with a spine.

ERAS mechanics

  • Write as many versions as you like, but each program receives only one.
  • Plain text only. No bold, italics or other formatting.
  • Copying text from example websites without credit counts as plagiarism.

A structure that works

Why this fieldOne real patient or moment, told briefly, that shows how you think.
What you've doneEvidence of commitment: projects, teaching, and clinical experience in the field.
Where you're goingA specific career direction, such as clinician-educator, research or a niche. Vague is fine; generic isn't.
Why you, why hereWhat you bring to a fellowship. Tailor the last paragraph for programs that matter most.
Skip — retelling your CV, the childhood origin story, and “I've always been fascinated by…”. One page, read aloud once, reviewed by someone in the field.

Signal with intent.

Who uses signals

ERAS program signals are open to fellowship applicants in participating specialties. The AAMC asks participating programs to use signals as part of holistic review when they decide whom to interview. Several IM subspecialties participate in the 2027 season. Nephrology, infectious disease and geriatrics are not on the 2027 list.

Signal where you would actually go — signals are scarce on purpose. Spend them on programs you want and could realistically match. Numbers change by season, so recheck the AAMC list before you submit. Under NRMP rules, programs may not ask you where you signaled.

Interview, then follow the rules.

Virtual is the default

AAIM recommends virtual fellowship interviews for all applicants, including residents at the program's own institution, and leaves the final format to each subspecialty society. In-person visits and second looks should be optional. Programs should commit to submitting their rank lists before holding any in-person second looks, so that visiting cannot affect your ranking. AAIM also asks programs to tell interviewees not to send thank-you notes. Clarifying questions are fine (guidance as of July 2025).

NRMP Match Participation Agreement (SMS, 2026)

You mayVoluntarily tell a program you're interested, and share your rank list with your own residency PD.
You may notAsk a program for a statement that implies a commitment to rank you.
Programs mayTell you that you're viewed favorably and will be ranked.
Programs may notAsk where else you applied or interviewed, where you signaled, or how you'll rank. They also may not make ranking depend on a statement of your preference.
Interview etiquette (6.2)Accept only interviews you intend to attend. Give about one to two weeks' notice to cancel or reschedule. Programs should give you at least 48 hours to accept or decline an invitation.
After the MatchThe result is a binding commitment. Backing out requires an NRMP waiver.
If asked where else you're interviewing — say you'd rather keep your list private, as the NRMP agreement allows, and, only if it's true, that this program is a top choice. The MPA (6.3) treats misleading information given to programs as a violation. Report persistent pressure to the NRMP.

The job search

There's no match for hospitalist or primary care jobs. You need a timeline of your own. Start conversations in the summer or early fall of PGY-3. Licensing, credentialing and payer enrollment can take months after you sign, and the best-fit groups often fill early.

Where to look

  • Your own hospital and the groups your attendings and alumni work in. Ask your chiefs who is hiring.
  • Specialty-society career centers: the American College of Physicians and the Society of Hospital Medicine both run job boards.
  • Health system career pages in the regions you want.
  • Recruiters can help with volume, but they work for the employer.

Restrictive covenantsNon-compete radius and length. Enforceability varies by state.

ExtrasCME money and time, loan repayment, and signing or relocation bonus with clawback terms.
On a J-1 visa? Waiver jobs run on a separate clock. Under the Conrad 30 program, each state can request up to 30 waivers a year, typically for at least three years of full-time practice in a shortage area. The program has a sunset date that Congress must periodically extend, so confirm it is currently authorized. State rules and windows differ. Talk to an immigration attorney early, and have a health-care attorney review any contract before you sign.
