Oscillometry measures one pressure and calculates the other two. Wrong size, wrong arm, wrong height, wrong number — and nothing on the screen will tell you that.

Oscillometry measures one pressure and calculates the other two.
What the machine actually measures
- Cuff pressure. Inflated, then bled down.
- The oscillations. Each pulse pushes back against the cuff.
- Their envelope. Amplitude peaks where cuff pressure equals mean pressure: that peak is the MAP. The other two are algorithm, not measurement.
Bladder width against arm circumference
| Fit | Effect |
|---|---|
| Too narrow, under 40% | The cuff squeezes harder to reach the artery. Overreads by up to 20 mmHg. The common error. |
| Correct, 40% | Width 40% of circumference, length 80–100% around, index line in range. |
| Too wide, over 50% | Pressure spreads along too much vessel. Underreads by 5–10 mmHg. |
Other sites
A forearm cuff overreads by 10–20 mmHg; size a thigh cuff to the thigh.
Position
Heart level, bare arm, supported; ten centimeters low adds 8 mmHg, and a loose cuff overreads. If the arms differ by more than 10 mmHg, use the higher.
Which number to act on
- MAP for perfusion targets and pressors; systolic for dissection and hypertensive emergency.
- Cuff against line. Level and flush the line first; if they still disagree in shock, believe the line.
- When it keeps failing, palpate a systolic, Doppler the radial, transduce if it matters.
Where oscillometry gives up

| Calcified vessels | Wall, not lumen: pseudohypertension. |
| Pre-eclampsia | Many devices underread here. |
| The wrong limb | Fistula, lymphedema, an infusion. |
